O32 Defining normal ranges for 96-hour wireless PH monitoring using healthy controls
نویسندگان
چکیده
Introduction Technology now allows up to 96-hour wireless pH-recording in clinical practice; however, no normal values exist for this methodology. This study acquired esophageal pH recordings healthy controls (HC) and compared these against measurements patients with endoscopic evidence of gastroesophageal reflux disease (GORD). Results were used validate the Lyon Consensus Classification GORD Diagnosis. Methods HC had monitoring (Bravo) over 96 hr at two tertiary centers. Bravo was inserted under sedation endoscopy. Median 95th percentile calculated acid exposure time (AET) 24, 48, 72 96-hrs ‘worst 24-hrs’ (i.e. most pathological day). The same analysis applied results from a database consecutive erosive esophagitis (Los Angeles (LA) classification) that completed 96-hr (acid suppressants stopped ≥5-days previously). A receiver operating curve (ROC) area ROC (AUC) Youden’s index also performed define optimal cut-off AET. Results 71 asymptomatic studied, whom 47 (age 28±9 years, 66% F) recording. (upper percentile) AET 1.7% (3.0%) any day 2.9% (4.5%) worst day. 136 (61 LA A, 60 B, 12 C, 3 D) - table 1. Linear regression revealed correlation (p<0.0001, R2=33%) between findings after adjusting gender, age duration test. average hrs differentiated group (LA sensitivity 92%, specificity 75%, positive predictive value (PPV) 77%, negative (NPV) 91% 3.1%, AUC 0.91. Similar present 24-hr’ (sensitivity 91%, 79%, PPV 80%, NPV 90% 5.8%; 0.92). Conclusions defines range ambulatory monitoring. These discriminated conclusive diagnosis, based on findings. diagnostic criteria proposed by Consensus. upper limit ~4% (3.0% AET, 4.5% AET) approximately ~6% (worst 5.8% AET).

Evaluation of pirenzepine on gastric acidity in healthy volunteers using ambulatory 24 hour intragastric pH-monitoring.
The effect of pirenzepine on 24 hour intragastric acidity was studied in 10 healthy volunteers using ambulatory 24 hour intragastric pH-monitoring in a double blind crossover study. Tests were performed on the seventh day of ingestion of either placebo, 75 mg pirenzepine or 150 mg pirenzepine per day. The drugs were given at two doses at 8.30 am and 8.30 pm. Mean nocturnal hydrogen ion activity...
متن کاملEstablishment of normal reference ranges for glycemic variability in Chinese subjects using continuous glucose monitoring
BACKGROUND Glycemic variability is increasingly recognized as an important issue in diabetes management. However, the lack of normative values may limit its applicability in the clinical setting. The objective of this study was to establish preliminary normal reference ranges for glycemic variability by analyzing continuous glucose monitoring (CGM) data obtained from healthy Chinese adults. M...
متن کاملValue of the 24 hour intraoesophageal pH monitoring in children.
Twenty four hour oesophageal intraluminal pH probe studies were performed in 114 children (range age: one month-12 years) referred for symptoms or signs compatible with gastroesophageal reflux. Forty five patients had reflux disease alone, 69 had evidence of oesophagitis which was assessed endoscopically and histologically. Recordings were also performed in 63 control patients. The occurrence o...
متن کامل24 hour ambulatory oesophageal pH monitoring in uncomplicated Barrett's oesophagus.
The aim of this study was to compare acid reflux in 16 patients with uncomplicated Barrett's oesophagus with 37 patients with reflux oesophagitis and 10 control subjects of comparable age. All patients had 24 hour ambulatory pH monitoring within one week of endoscopy. The controls differed significantly from the other groups in all indices of reflux but there was no difference between the Barre...
متن کاملEsophageal and Gastric Diseases Ambulatory 24 - hour Esophageal pH Monitoring
The incidence of gastroesophageal reflux disease (GERD) is increasing and if left untreated can lead to significant patient morbidity and even death. The disease results from the abnormal reflux of gastric contents into the distal esophagus causing symptoms in most and subsequent mucosal damage in some. Several investigations can be used to confirm the diagnosis, but most are dependent on the p...
